I, the undersigned, am the owner or authorized caregiver of the above-named pet and have voluntarily chosen to use the services provided by Bark Valley Stables. I acknowledge and agree to the following terms and conditions:

I release Bark Valley Stables, its owners, employees, Carlie Dishman, and Isabella Crowder from any liability for any injury, illness, or damage that my pet may suffer during or as a result of the services provided, including any transportation to or from the facility. This release of liability includes, but is not limited to, injuries or illnesses resulting from dog fights, accidents, or contagious diseases.

I understand and acknowledge that there are inherent risks associated with dog boarding, daycare, grooming, and related services provided by Bark Valley Stables, including but not limited to the risk of injury, illness, or death.

I authorize Bark Valley Stables to seek emergency veterinary care for my pet if deemed necessary by their staff. I understand that I will be responsible for all associated costs.

I confirm that my pet is up-to-date on all required vaccinations including Rabies, Bordetella, and DHLPP. I confirm that I will provide proof of vaccinations upon request.

I grant Bark Valley Stables, Isabella Crowder, and Carlie Dishman permission to use photos or videos of my pet for promotional purposes.
